Information about Salary Allotment Coverage

1. There must be at least 60 regular employees in the company, and at least 10 application forms must be submitted for initial processing.

2. Individual application form and one (1) 1 x 1 recent ID picture must be submitted by each enrollee together with two (2) copies of Authorization for Allotment of Pay Form which may be secured from the Marketing Department.


3. Authorized officer of the company must submit an acceptance letter signifying their permission to install the facility to their company.

4. Enrollees are allowed to choose their own plan. Monthly mode of payment is applicable. No enrollment fee is required. Salary allotment (SA) rates shall apply.
Individual agreement/policy and ID card will be issued.
