ACCESS PROCEDURES
1. Know the scope of your health plan. Understand the services you may avail of, procedures for availment and limitations of your plan.
2. Have your PhilCare ID handy at all times. You need to present your ID when you avail of the health care services under your plan.
3. Take care of your PhilCare ID. It would be very inconvenient for you if you lose your ID. It is a quick reference of your health plan and is always a requirement for availment of services.

CLAIMS / REIMBURSEMENT PROCEDURE
1. Secure Reimbursement Claim Form (RCF) from PhilCare
2. Accomplish the Reimbursement Claim Form
- Member fills out Part I of the RCF
- Attending Physician fills out Part II of the RCF
- Client Company fills out Part III of the RCF
3. Submit the following documents to PhilCare (within 60 days from date of discharge)
